Medical history data refer to the patient and can be entered and edited any time.

Medical history data are always associated with personal data.

Example:

Medical history data are constructed in the form of a tree, displayed on the left-hand side of the window.

The data consist of several pages that can be viewed and edited in two ways:

• by clicking the desired page, in the tree displayed on the left-hand side of the window
• by clicking the buttons < Previous and Next > to go to the page coming immediately before and after the page on display.

Types of data

Data can be of the following kinds:

• close-ended, e.g. when it is possible to choose Yes or No.
• free text, e.g. in the fields made available to enter additional remarks

A form can be printed and passed out to patients so that they can fill it in by hand. The data collected can be entered on the computer program at a later stage (see print a blank form to be completed by hand).
